Health Navigation in Alachua County

At a Glance & Program Features

Health Navigation works directly in home with clients and caregivers developing a health service plan to meet the client's health and disability service needs. The Health Resource Navigator provides linkages regarding the health services available in the community, assists the client in gaining access to services, and aids families in obtaining available funding sources for continued health care. The program serves clients 18-65 years of age who are at or below 150% Federal Poverty Level.
